THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

My commitment to your privacy

The law requires me to keep your health information private. “Protected health information” (PHI) means anything that identifies you and relates to your health, the services I provide, or payment for those services. This includes your name, phone number, email address, test results, and reports.

I am required to:

  • Keep your PHI private and secure.
  • Give you this notice explaining my legal duties and privacy practices.
  • Follow the terms of the notice currently in effect.
  • Tell you promptly if a breach occurs that may have compromised the privacy or security of your information.

I use or share only the minimum information needed for each purpose. When California law protects your information more strictly than federal law, I follow California law.

How I use and share your information without your written permission

Treatment. I use your information to evaluate you and make recommendations. With your permission, I share it with other clinicians involved in your care.

Payment. I use your information to bill and collect for my services. For example, I may send your health plan the information it needs to approve or pay for an evaluation.

Running my practice. I use your information to run my practice well, such as reviewing the quality of my services. I may share it with attorneys, accountants, or consultants who help me follow the law.

Service providers (“business associates”). I use outside companies for my electronic health record, appointment reminders, secure email, text messages, and billing. Each one signs a written agreement requiring it to protect your information and use it only to provide services to my practice.

Appointment reminders, forms, and billing notices. I may contact you by phone, secure email, or text message to remind you of appointments, ask you to complete forms, or tell you about a balance due. The section “Email and text messages” below explains how.

When the law requires or allows it. In limited situations, I may share information without your permission:

  • When required by law, including court orders and subpoenas that meet legal requirements.
  • To report suspected abuse or neglect of a child, elder, or dependent adult.
  • To prevent or lessen a serious threat to the health or safety of you or others. This includes my legal duty to warn when a patient communicates a serious threat of violence against a reasonably identifiable person.
  • For public health activities and health oversight, such as audits or licensing investigations.
  • For lawsuits and legal proceedings, as allowed by law.
  • To law enforcement, coroners, or medical examiners, as allowed by law.
  • For workers’ compensation, as required by law, if you are evaluated as part of a claim.
  • For specialized government functions, such as military or national security purposes, as allowed by law.

Uses that require your written permission

I will not do the following without your written authorization:

  • Share your information for any purpose not listed above.
  • Use or share psychotherapy notes, except in the limited cases the law allows.
  • Use your information for marketing.
  • Sell your information.

You may cancel an authorization at any time by writing to me. Cancelling stops future sharing but cannot undo sharing that already happened.

Test materials and raw test data

As required by the APA Ethics Code and test publisher agreements, I protect the security of test questions, test forms, and scoring materials. If you request your records, I will provide your report and information about your results. Raw test data are released to you or another qualified professional as the law and professional standards allow.

Email and text messages

I communicate with patients by HIPAA-compliant secure email and by text message. Each is used only for the purposes listed here.

Text messages are sent only for:

  • Appointment reminders and scheduling
  • Requests to complete forms
  • Notices that a balance (such as a deductible) is due

Text messages contain as little health information as possible. They will not include diagnoses, test results, or clinical details. Forms and bills are completed through secure links.

Secure email is used for appointments, forms, billing, and delivering an electronic copy of your final report. Emails that contain your report or other health information are encrypted.

Your choices. Texting and email are optional. You may opt out of text messages at any time by replying STOP, or ask me to stop email by calling 530-750-1700. You can then receive these communications by phone or mail instead. Do not use text or email for emergencies. In an emergency, call 911 or go to the nearest emergency room. For a mental health crisis, call or text 988.

No sharing of your mobile information. I do not sell, rent, or share your mobile phone number or text messaging consent with third parties or affiliates for marketing or promotional purposes. Information may be shared only with the service providers that help me send messages, and they are bound by confidentiality agreements. The above excludes text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Information collected through this website

This website collects only what you choose to send, such as information you enter in patient forms or a contact request. Information you submit about your health is treated as PHI under this notice. The site may use basic cookies needed for it to work; I do not use them to track you for advertising. Payments made through PayPal are handled by PayPal under its own privacy policy, and I do not receive your full card or bank details.

Your rights

You have the right to:

  • See and get a copy of your records, on paper or electronically. I may charge a reasonable, cost-based fee. I will respond within the time limits set by law.
  • Ask me to correct your records if you believe they are wrong or incomplete. If I say no, I will explain why in writing, and you may file a statement of disagreement.
  • Ask for confidential communications, such as contact at a different phone number or address. I will agree to reasonable requests.
  • Ask me to limit what I use or share. I am not required to agree, except in one case: if you pay for a service in full yourself, you may ask me not to share information about it with your health plan, and I will agree unless the law requires sharing.
  • Get a list of certain disclosures I have made of your information in the past six years.
  • Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you. A legal guardian or someone with medical power of attorney may exercise your rights.

To use any of these rights, contact me at the phone number or address at the top of this page.

Changes to this notice

I may change this notice. Changes will apply to all information I hold. The current notice will always be posted on this website and available in my office, and I will give you a copy on request.
